One and one-half times annual salary up to $50,000 up to $250,000.  May purchase additional in increments of $10,000 on a fee schedule based on age.  Additional family coverage optional.

Short-term Disability

1st of the month following one year of employment

Eligible after 7 days of injury or illness.  Pays 50% of weekly salary to $300.00, Buy-up option available for up to $750 weekly coverage

Long-term Disability

Part-time (32 hrs. per week)

 

First of the month following one year of employment and after 6 months of short-term disability.

Pays 66 2/3% of salary to $6,000 per month.
